Near MINT condition 5 hook Heddon Underwater Minnow model 150 in Rainbow color with the correct box. I hate to say mint as nothing is absolutely perfect but this looks to have never been fished and i can't find anything wrong with it. You will never find one better and will never a need to upgrade.
